CCHAMP Center of Excellence

CCHAMP, which stands for Chronic Care HIV/AIDS Multidisciplinary Program, is a Center of Excellence operated by Zuckerberg San Francisco General Hospital (SFGH). The program provides case management for people receiving HIV services at the SFGH HIV clinic, Ward 86.

CCHAMP provides:

  • One-on-one support from a case manager
  • Access to three closed support groups per month
  • Assistance and support to stay engaged in HIV care
  • Referrals to community resources including social and health services

CCHAMP is available to people receiving HIV care at Ward 86 by referral only. Talk to your healthcare provider if you are interested in receiving CCHAMP services.
